How it runs
Discovery first, and it is about your process rather than your requirements. The requirements list usually describes the current system; the process is what the new one has to serve.
Then a build against a written acceptance checklist. One row per thing the system must do, in your words. You test each one yourself and nothing goes live until every item has passed and you have signed. That gate is enforced by the software, not by goodwill: the signature is refused while anything is outstanding.
After launch, 45 days of hypercare at no extra charge, then a support arrangement or a clean handover with the documentation to run it yourselves.
What it costs to get wrong
The expensive failure is not a bug. It is a system that does what was specified and not what was needed, discovered at the end, when the budget is spent and changing it is a new project.
